I, individually and on behalf of my heirs, successors, assigns and facilities, do hereby fully release and forever discharge Chen Village Tai Chi, certified teachers, authorized teacher trainees, their officials, agents and volunteers from any and all claims for injuries, death, damages or loss including transportation services if provided, that I may have or which may accrue to me and arising out of, connected with, or in any way associated with participating in any and all classes at Chen Village Tai Chi.

I have read and fully understand the above waiver and release of all claims by typing a full name above as electronic signature.

I have also received approval to participate in any and all classes from a physician's by signing above.
